Perinasal fixation is not a standard technical term in the field of cosmetic medicine. It refers to a surgical procedure that fixes and reshapes the area around the nose (including the alar, tip, and base), commonly seen in rhinoplasty. Through cartilage grafting, suture fixation, or supporting structures, the shape of the nasal tip and alar can be stabilized, preventing postoperative deformation or retraction, thus achieving a natural and lasting effect.

Core surgery: Perianasal fixation primarily addresses issues such as drooping nasal tip, wide nasal alar, and nasal bridge swelling. Common surgeries include:

Rhinoplasty/Tip Reduction: Adjusting the alar cartilage and suturing the nasal tip to make it more pointed and defined. This is often combined with cartilage grafts (such as ear cartilage) as a supporting structure to enhance stability.

Alar reduction surgery: Removes excess tissue and sutures the alar to reduce the width of the nostrils and improve the appearance of the alar when smiling.

Septal lengthening surgery: Cartilage is transplanted and fixed to the nasal septum to lengthen the columella, lower the nasal tip, and create an upturned nose shape.

Cartilage graft fixation: Ear cartilage or rib cartilage is transplanted to the tip/columella of the nose to act as a "scaffold" to stabilize the nose shape and prevent degeneration caused by soft tissue contraction.

These procedures are often combined in a comprehensive rhinoplasty to address multiple perinasal issues in a single surgery. Post-operatively, the nose is immobilized using plaster casts, tape, or internal sutures to ensure stability.
